- Documentation needed at time of Orientation

• Current profession License on CNA certification
• Social security care or Birth certificate
• Current employment Authorization card (if appropriate)
• Current Drivers license or State ID card
• Current auto insurance card
• Physicians statement of good health stating (free of communicable disease)
• Negative TB skin test results. If you have tested positive we need chest x-ray report.
• CPR certification
• Date of Hepatitis B injections if applicable.
• For Facility staffing: NRP, PAL, ACLS (depending on specialty)

These items are required before you will be able to work.
